– Employee bathroom door is not self closing.
– Fan covers in walk-in cooler have build up of dust.
– Facility is operating without a processing license for wholesale. License expired March 31, 2016. Corrected on site: license application filled out and check mailed to office.

Carol Lee Donuts, 104 S. 5th St. (May 26) Modified Complaint Inspection
– No Violations.
– Complaint not valid.
